 Note for INITIAL STATE LICENSES:
The following Arizona Revised Statute and Arizona Administrative Code rule will direct the scheduling of initial state licensing surveys:

1. ARS 36-422.C requires

- An application for licensure shall be filed at least sixty but not more than one hundred twenty days before the anticipated operation;

2. AAC R9-10-108 (Time-frames for Initials)

- Overall Time-frame for the Department to review the application and conduct the initial licensing inspection is 120 days.

- Once the application is considered administratively complete, including the ready for survey notification, the initial licensing survey will be placed on our schedule. We will conduct the initial inspections in order they are received and determined to be complete.
