On July 27, 2006, CMS proposed a 3.1 percent increase in Medicare payment rates to home health agencies for calendar year 2007.  The proposed increase would bring an estimated extra $420 million in payments to home health agencies next year.  As part of this proposed rule, CMS is proposing pay-for-reporting provisions of the Deficit Reduction Act of 2005.  In addition, CMS proposed changes to Medicare payment for oxygen equipment and capped rental durable medical equipment due to the Deficit Reduction Act of 2005.

National Provider Identifiers (NPI) are Free!!  Has your agency gotten its NPI yet?  As the industry transitions to NPI compliance, remember that there is no charge to get an NPI.  Providers can apply online for their NPI, free of charge, by visiting https://nppes.cms.hhs.gov or by calling 1-800-465-3203 t request a paper application.  The CMS NPI page, located at  www.cms.hhs.gov/NationalProvidentStand/  is the only source for official CMS education and information on the NPI initiative.

HHAs must begin using the new Home Health Advanced Beneficiary Notice (HHABN) exclusively no later than September 1, 2006.  The form has been approved by CMS for use through August 31, 2009.

Good News!  CMS has revised the Conditions of Participation regarding the encoding and transmitting of OASIS data.  Effective June 21, 2006, CMS is no longer requiring HHAs to encode OASIS data within 7 days of the assessment completion date.  The new requirement states HHAs will have 30 calendar days from the date the assessment was completed to both encode and transmit completed OASIS data.  Click on the link to read the final rule. Today the Centers for Medicare & Medicaid Services (CMS) announced proposed changes to the Medicare Home Health Prospective Payment System (HH PPS) that will improve the accuracy of payments to home health agencies for services they furnish to Medicare beneficiaries.  The display copy of the proposed rule is posted on the CMS website at: http://www.cms.hhs.gov/HomeHealthPPS/downloads/CMS-1541-P.pdf.

This rule includes a number of refinement proposals that will update the case-mix model to better reflect the current mix of home health patients, better reflect resources associated with certain type of episodes, and adjust for case mix creep.  In addition, this rule proposes routine annual updates to the market basket, the fixed dollar loss ratio, and pay for reporting requirements.  This update reflects the proposed home health market basket of 2.9 percent for Medicare payment rates for calendar year 2008 as well as a 2.75% reduction in the rates to account for nominal change in case-mix.  The overall impact of the proposed refinements of this rule is estimated to increase total payments to home health agencies by approximately $140 million in CY 2008.  For more detailed information on the provisions of this proposed rule, refer to the HHA Fact Sheet http://www.cms.hhs.gov/apps/media/press/factsheet.asp?Counter=2134

CHANGES IN OASIS TRANSMISSION IS ALMOST HERE

The old modem to modem over a telephone line method of transmitting OASIS data is soon to be no more.  CMS has begun switching from AT&T’s dialer up service to broadband.  Broadband is what many agencies already use to access the internet. 

Arizona is scheduled to be switched to broadband starting August 7th.  In order to begin using broadband transmission, each agency will need to update their AT&T Global Dialer to version 7.0, version date 5/24/07.  You can check which version of the dialer you have by opening the dialer and left-clicking on HELP located on the top menu bar; then selecting ABOUT.  The version number and date will be displayed. NOTE:  If you have Windows Vista on your computer, you must have the 7.0 version of the dialer.  Version 6.9 will work on all Microsoft operating systems other than Vista.

You can obtain a copy of the latest version of the dialer, and installation instructions, by going to www.qtso.com and clicking the MDCN Information link inside the blue outlined box on the right hand side of their home page.  This new version of the dialer might take some time to download because it is a large file.  You can request a CD version of the dialer be mailed to you by emailing a request to mdcn.mco@palmettogba.com.

If your agency is on a corporate wide area network (WAN) or a local area network (LAN), your computer specialist or IT (Information Technology) person will probably have to configure your network to allow access to the broadband connection through the new dialer client.  The instructions for network installs are provided at www.qtso.com in the same location at the dialer download.

If you are able to install the new dialer client and do not need the assistance of AT&T, you may begin using the broadband method of transmitting immediately.  However, if you need assistance from AT&T, you will have to wait until August 7, 2007 to call them.  The Help Desk number is 800-905-2069.

If broadband internet access is available in your area, you will not be allowed to continue to use the current transmission method of modem to modem dial-up.  CMS is requiring agencies in areas where there is no broadband service available to submit a waiver request to continue using the current phone modem method.  The waiver from is also available on the www.qtso.com site.

TOY GROUPER

The Centers for Medicare & Medicaid Services (CMS) is providing a home health temporary (toy) grouper based on the refinement proposals in CMS-1541-P (Home Health Prospective Payment System Refinement and Rate Update for Calendar Year 2008). This tool is not the official grouper software that CMS will add to HAVEN (the software provided free from CMS for Home Health Agencies to use to submit their OASIS data).  It is a practice tool that can be used for educational and planning purposes.  Also, CMS is providing “pseudocode” that provides detail on the logic of the new classification algorithm to help software vendors to begin preparing for the revisions that will be required to their programs and systems.  Please refer to the following documents for more information on the temporary (toy) grouper and the pseudo code:
