Rates and charges for
ambulance services in the State of Arizona are set by the Director of
the Arizona Department of Health Services by a hearing process
delineated in A.R.S. § 36-2234. The Bureau of Emergency Medical
Services, Ambulance and Regional Services Section, serves as
facilitator to ambulance companies seeking initial rates and charges
and/or adjustments to their current rates and charges.
Automatic
Rate Increase
The Calendar Year 2008 Automatic Rate Increase is 3.57%. To apply,
please provide the Bureau with a Letter
of Application [DOC 22K].
General
Rate Increase
To apply for the General Rate Increase, please provide the Bureau with a
Letter and Application Packet [DOC 23K]. The packet inclusions are identified in Arizona Administrative
Code R9-25-1102.B [DOC 20K].
Arizona
Ground Ambulance Rates
The Ground Ambulance Service Rate Schedule [PDF 18K] lists the base rates for
Advanced Life Support (ALS), Basic Life Support (BLS), Mileage,
Standby/Waiting and Subscription Service (in order to download the PDF
version Acrobat
Reader™ is required). The Rates Effective Date
describes when the last adjustment was made to the rates and charges of
a particular ambulance company.
Ambulance
Revenue and Cost Report Forms
Pursuant to
A.R.S. §
36-2232.A.6, an ambulance service operating under a Certificate of
Necessity (CON) is required to file annual financial reports not later
than 180 days after the completion of its annual accounting period.
The Bureau of Emergency Medical Services requires ambulance services to
report financial data using the Ambulance
