The following disciplinary/enforcement actions have been taken by the Arizona Department of Health Services, Bureau of Emergency Medical Services & Trauma System against Arizona state certified Emergency Medical Technicians (EMTs) in 2003.

Probation Orders are not listed individually, but a list of names and Probation Orders can be reviewed at the Phoenix BEMSTS office during business hours.

Approximately 13,000 EMTs are certified in Arizona.  Many EMTs share the same names.  The list of disciplinary/enforcement actions should be read carefully for names and certification numbers.
