What committee membership vacancies currently exist and how can I get appointed to a committee?

To determine committee vacancies, view the Committee Rosters available on the Committees' webpage.

Each committee's responsibilities are identified on the Committee Roster. If you determine a committee(s) that you are interested in, please send a letter of interest to the Bureau Chief, Bureau of EMS, 150 North 18th Avenue, Suite 540, Phoenix, AZ 85007.

How can I suggest a rule change?

To suggest a rule change to rules pertaining to emergency medical services, please contact Maria Herbert, BEMS Rules Coordinator, at (602) 364-0912 or via email.

How can I request an investigation?

The Bureau of Emergency Medical Services is required to receive written or electronic complaint forms in order to open an investigation.

The Enforcement Section may only investigate issues which appear to show the existence of a violation of Arizona Statute or Rule.

You may submit our complaint form [PDF 51K] [DOC 69K] to the Bureau by traditional mail to Enforcement Section, Bureau of Emergency Medical Services, 150 N. 18th Avenue, Suite 540, Phoenix, Arizona 85007-3248, or by attaching the completed document to an email and sending it to our Enforcement Section.

An investigator will contact you for further information during the course of the investigation process.  (A.R.S. § 36-2211-B; A.R.S. § 36-2245-C)

How can I receive a Do Not Resuscitate (DNR) Form?

To receive A Do Not Resuscitate (DNR) Form (also known as Prehospital Medical Care Directive), please call the BEMS Administrative Offices at (602) 364-3150 or send a request via email. Additional information concerning the DNR form is available on this website
