How can I file a complaint against a trauma center?

The Trauma Services Section of the Bureau of Emergency Medical Services (“BEMS”) can receive a complaint in written, telephonic (telephone or facsimile), or verbal form to open an investigation.  Only those issues that appear to show the existence of a violation of applicable Arizona Statute or Rule may be investigated.  Issues that appear to show the existence of a violation of non-applicable Arizona Statute or Rule, but that of another Arizona Department of Health Services program or other state agency will be appropriately referred.

A Trauma Services investigator will contact you for further information during the course of the investigation process.
