| The Arizona
Department of Health Services (Department) receives many requests from the
public for records disclosure pursuant to Arizona's Public Records Law
A.R.S. §§ 39-121 through 39-127. The Department can release a
public document but not any confidential information contained within
the public document. The confidential information must be redacted.
The Department also receives subpoenas for public records from attorneys for use in
administrative or court proceedings. If the records contain
medical information, the medical information must be redacted unless the
Department receives a valid authorization signed by the individual whose
information is contained in the record or the individual's
guardian.
