Employee Recognition Program

At the Arizona Department of Health Services (ADHS), we believe that our employees are our most important asset.  Our commitment to excellence is realized through the contributions of individual employees as well as the collaborative effort of teams.

The ADHS Employee Recognition Program was established in 1992 and was created to celebrate and support employees through recognition. The Department’s Employee Recognition Coordinator is Michelle Borquez, who is responsible for coordinating Department-wide employee recognition programs and activities.  She also chairs the Employee Recognition Council, a group of dedicated employees who promote employee recognition activities throughout the agency.

The Department encourages you to recognize these individuals by nominating them for their customer service, teamwork, integrity, or overall positive attitude. If you’d like to nominate an employee or a particular team or office for the service they provided you, just fill-out a nomination form.  In order to view the PDF version, Acrobat Reader is required
