Managing for Excellence Program
Community Health Assessment
The Managing for Excellence Program (MEP) has partnered with each of the county health departments to conduct a Community Health Assessment (CHA). The purpose of the CHA is to learn about the health status of Arizona’s population. The CHA is a collaborative process of collecting and analyzing data from the community and information from a variety of sources.

State Level Health Assessment
ADHS is in the process of completing a public health assessment report at a state level, the 2013 Arizona State Health Assessment (SHA). The SHA is one of the three PHAB accreditation prerequisites, and will be the basis for the State Health Improvement Plan. ADHS is partnering with local health departments and other public health organizations to accomplish this collaborative effort.
