http://www.azdiabetes.gov/
- The goal of the program is to reduce the burden of diabetes in the state of Arizona. Much of this burden can be prevented with early detection, improved delivery of care, and better education on diabetes self-management. The program targets at-risk populations, persons living with diabetes, and their families.
- The program provides opportunities for diabetes training for professionals and community health outreach workers to promote best-practices and standards of care for the treatment of people with diabetes. The program also has a coalition, a 400-member group made up of organizations and individuals interested in diabetes, and the Arizona Diabetes Leadership Council, an 18-member advisory council made up of diabetes experts and leaders from throughout the State. The council and coalition work together to collaborate with partners in the State to develop community interventions, health communications programs, and strategies to make changes in health systems that will improve the care and treatment for people with diabetes; promotes wellness and physical activity for people with or at risk for diabetes; and addresses health disparities through training of professionals and community health outreach workers and outreach activities.
