Loan Repayment Application – Reapplication/Renewal
A reapplication form is available for an exisitng loan repayment recipient who wishes to renew his/her loan repayment contract and whose personal/provider and service site information is still accurate as in the original approved application other than loan balances and repayment amounts. Otherwise, an applicant must re-apply by submitting a complete application packet.
The deadline for submitting renewal applications follow the same deadline as new applications. Loan repayments providers requesting for an extension must ensure timely submission of their renewal applications in order to prevent any gap in service.
Applicants who have not previously registered with ProcureAZ must register for consideration.
