Service Site Change – Additional Site Request

Employers who wish to assign a J-1 provider to an affiliated service site in addition to the original approved site must notify the Arizona Department of Health Services (ADHS) prior to this assignment. ADHS must document that the additional service site(s) meets the service site eligibility requirements of the Arizona J-1 Visa Waiver Program through the submission of the Additional Service Site PacketPDF for the new site or sites. To compile the required documents, review the Additional Service Site Checklist.PDF ADHS will inform the J-1 physician or the representing attorney of additional supporting documents needed to ensure that the additional service site meets program requirements.

The J-1 physician or his/her representing attorney must file a petition to amend his/her J-1 status directly with the United Citizens Bureau of Citizenship and Immigration Services (USCIS). Once this petition is approved, the J-1 provider must submit a copy of the approval notice to ADHS for documentation.
