Part A - Laboratory

This form is required for all initial and renewal applications and
whenever there is a change in owner or director or facility name.
Changes in director or owner or facility name must be submitted within 20
days of the change to the Office of Laboratory Services.
Part B - Laboratory Personnel

This form is where the laboratory may select which parameters to be
licensed for by Arizona.

director of the Department after the rules were promulgated.

Initial Application
Arizona licensure requires a completion of the application per
A.A.C. R9-14-603.A. In addition, the Office of Laboratory Services will
conduct an on-site inspection of in-state laboratories within 30 days and
out-of-state laboratories within 60 days.
Renewal Application
Arizona requires annual renewal of the application (along with
the fees). In the renewal process, if the laboratory does not wish to make
changes to their license, may submit Part A and Part B only.
