1.  What is CLIA?

    CLIA is the Clinical Laboratory Improvement Amendments and is regulated by the
    federal government through the state agency, the regional office and the central office.

2.  Where do I apply for a CLIA certificate?

    A laboratory may apply for a CLIA certificate through the Office of Laboratory
    Services (OLS), CLIA Program.  Call (602) 364-0726 or
    write to 250 N. 17th Avenue, Phoenix, AZ 85007-3231.

3.  Where can I find CLIA regulations?

    CLIA regulations are available through the CMS link new.cms.hhs.gov/clia or through
    the Code of Federal Regulations, 42 CFR 493.

4.  Where do I send my CLIA payments?

    Payments are sent to:  CMS Lab Program
                                    P.O. Box 70948
                                    Charlotte, NC 28272-0948
    Checks should be made out to CMS Laboratory Program and should include the CLIA 
    number on the check to ensure that the payment is accredited to the correct
    account.  Do not send payments to the state.

5.  Can I send only the application to CMS?  to the state?

    No, the state has additional forms that must be completed to fulfill program
    requirements.  Please contact (602) 364-0726 for more information.

7.  Does Arizona license laboratory testing personnel and phlebotomists? 

    No.  The state does not license testing personnel or phlebotomists.  Employers
    determine qualifications.
