Application for Initial Licensure

Call (602) 364-0720, Monday through Friday from 8:00AM to 5:00PM.

Pursuant to A.R.S. § 36-495.03(A), a laboratory must file an application with the department at least thirty days before the anticipated operation of a new laboratory for an environmental laboratory license accompanied by the license application fee.

Application for Renewal Licensure

ADHS as an office policy sends out to all licensed laboratories a blank application 90 days prior to an out-of-state laboratory's license expiration and 60 days prior to an in-state laboratory's license expiration.

Pursuant to A.R.S. § 36-495.03(F), A regular license expires one year after the date of issuance and shall be renewed on submission of a renewal application and payment of the renewal application fee prescribed in A.R.S. § 36-495.06, at least thirty days before expiration of the license, unless the director determines pursuant to A.R.S. § 36-495.09 that grounds exist to deny the application.

  2. Technical Licensure Information - Call (602) 364-0720 and ask for either an environmental laboratory consultant or a member of the Training and Technical Resources staff.
  3. System or Permit Testing Requirements - Call ADEQ at (602) 771-2300 or (800) 234-5677 to be transferred to the appropriate section.
