Arizona List of Licensed Laboratories with Parameters - A list of all laboratories licensed by the state of Arizona for testing of drinking water, wastewater, solid waste and air.
Statutes (Title 36 Chapter 4.3) and Rules for Arizona Environmental Laboratory Licensure
Application for Licensure - Instructions and forms for applying to become licensed in the state of Arizona.
Environmental Laboratory Advisory Committee (ELAC) - The committee meets quarterly to discuss rules and other technical issues affecting the licensed laboratories and various regulatory agencies.
Checklists, Spreadsheets, Example Bench Sheets, and SOP Templates
The ADHS Environmental Laboratory Licensure program has made every effort to ensure the accuracy of the information provided in the following checklists, spreadsheets, and bench sheets. The following are to be used as tools to assist facilities in performing the approved methods correctly. Please read the disclaimer prior to using any of the spreadsheets or bench sheets. The checklists are provided as examples of what surveyors will be looking for when performing an on site visit.
The Quality Assurance/Quality Control document is required for all wastewater exempt methods compliance testing.
Temperature Form – Example of a form used for taking temperature of refrigerators and other equipment
Thermometer Calibration Form – Example of a form used when calibrating thermometers
Method Detection Limit or MDL Spreadsheet – Used to calculate MDL values
Linear Regression Spreadsheet – Used to evaluate and calculate linear calibrations
Demonstration of Capability - Used to calculate capability for operators during training of exempt methods
Bench Sheets These are example bench sheets for the exempt methods. Please note that there are two workbooks for each method, one for a single day worth of testing and one for multiple days.
* These bench sheets were provided and posted with permission from the Pima County Regional Wastewater Department and Laboratory. The acceptance criteria of 90-110% for the standard check are not required by the method and facilities may change those criteria, as an example a facility changed the recovery to 70-130%.
SOP Templates
These are standard operating procedure (SOP) templates for the exempt methods. Please note that the templates are written for the most common exempt method seen in the field, but these templates can be used for other methods as long as the facility updates the method specific requirements.

Training Information
ADHS provided training in late spring 2010, late summer 2010, and August 2011, and has posted the training presentations and/or information below. If you have any questions, please do not hesitate to contact our Office at (602) 364-0720. These training classes provided basic information on the regulations and detailed discussions and demonstrations of the approved methodologies.
ADHS may provide additional training in the future, which will be posted here or you may contact our Office to be placed on a notification list.
ADHS has performed three rounds of wastewater plant training (AZPDES/APP/Reuse). These trainings occurred in Spring 2010, Fall 2010, and August 2011. The following presentations and videos were presented in these trainings and should be used as a guide for meeting permit compliance. Except where noted, presentations apply to any permit required to report compliance data to ADEQ (AZPDES/APP/Reuse).
The following presentations and videos were given at a one day seminar provided in January 2012 specifically for mines with AZPDES and APP permits. Please note that these presentations apply to all permit types (AZPDES/APP).

Please note that those methods that are available free of charge are provided on this page, all other methods are considered proprietary and must be purchased from the organization or company.
