Chronic diseases such as heart disease, stroke, lung disease, cancer, diabetes and asthma are among the most common, costly and preventable of all health problems in Arizona. Chronic diseases are responsible for 7 out of 10 deaths among Arizonans each year, and account for 75% of health spending. These diseases are often preventable and can be managed effectively when detected early. While many factors can contribute to chronic diseases, it is important to understand that you can greatly lower your risk of developing a chronic disease, and prevent or delay the onset of disease by not using tobacco, maintaining a healthy diet, staying physically active, and getting annual health screenings that are appropriate to your age and other risk factors.

Tobacco Free Arizona
Tobacco Free Arizona strives to build individual, group, and community capacities to reduce the impact of commercial tobacco abuse. The goal of Tobacco Free Arizona is to use local and statewide resources, groundbreaking methods, and model national best practices to create a tobacco free future for Arizonans!

Arizona Diabetes Program
The Arizona Diabetes Program provides education and awareness of diabetes and its complications. The program monitors the burden of diabetes and its complications in Arizona, implements programs that address the burden of diabetes and develop partnerships through the Diabetes Coalition.

Arizona Heart Disease and Stroke Prevention Program
The Heart Disease and Stroke Program (HDSP) aims to increase the cardiovascular health of all Arizonans and decrease the burden of heart disease and cerebrovascular disease through health education.

Arizona Living Well (AzLW) Program
The ADHS Arizona Living Well Program has used funding from the Administration on Aging to join with other community partners to establish the Arizona Living Well Institute (AzLWI). The AzLWI was created to provide the infrastructure and capacity to disseminate evidence based health promotion programs across Arizona.
