To maintain and protect our
health as we age it is important to have access to and utilize all of the
preventive health screenings that are available through health care providers.
When planning healthy aging programs or services focused on chronic conditions,
there are resources available for HACN partners. Early detection is important.
There is a one time
“Welcome to Medicare” physical exam that includes preventive screenings and you can link
to “
Your Guide to Medicare’s Preventive Services" for more information.
Reducing risk factors for chronic conditions and diseases such as
physical inactivity,
smoking and
poor nutrition are also important.
If a chronic disease has already been diagnosed it is important for each older adult to understand and take an active role in the management of the chronic condition.

& tools such as evidence-based programs.
The HACN provides resources for the Stanford Chronic Disease Self-Management
Program, resources and connections to partners addressing chronic diseases such
as
diabetes,
cardiovascular disease,
arthritis, osteoporosis and
cancer.
The HACN has a
Chronic Disease Self-management Program (CDSMP) Master Trainer's group which meets
regularly via the web with their partners offering this program in the aging
