Arizona schools must submit an annual Immunization Data Report (IDR) on students enrolled in preschool, pre-k, kindergarten, sixth and tenth grades by November 15th each year. Childcare centers, small group childcare homes and Head Start programs must submit annual Immunization Data Reports on children born on or after October 1, 2006.
School IDR Forms for 2011-2012 School Year
Online IDR forms for kindergarten, sixth and tenth grades will be available in late September 2011 for schools in Apache, Cochise, Coconino, Gila, Graham, Greenlee, La Paz, Maricopa, Mohave, Navajo, Pinal, Santa Cruz, Yavapai, and Yuma counties.
Schools located in the 14 counties listed above will enter their immunization information into Immunization Data Report (IDR). To prepare for the report, please find the CTDS number for your school.
The CTDS number will need to be entered into the online IDR as a unique identifier for the school.
NOTE: Private schools do not need a CTDS number. A list of questions that will be included in the Immunization Data Report for each grade level is posted below:
NOTE: Only Pima County schools will receive paper forms from Pima County Health Department. Please contact Pima County Health Department, Vaccine Preventable Disease Program, at 520-243-7988 if you do not receive forms in the mail.
Childcare, Preschool, and Head Start IDR forms
Immunization Data Report forms for childcare, preschool and Head Start centers will be mailed in August and September 2011. Maricopa and Pima County centers should contact County Health Departments regarding their forms. Centers in all other counties will receive their forms from Arizona Department of Health Services.
Childcare facilities are welcome to print and complete copies of the forms listed below. NOTE: Please keep a photocopy of each form you submit.
