Office for Children with Special Health Care Needs
Service Coordination
 

boy with SantaThe Office for Children with Special Health Care Needs contracts with providers who are available to support families within each county of Arizona.  Service Coordinators network with insurance companies, the Arizona Department of Economic Security (DES), Arizona Health Care Cost Containment System (AHCCCS), Arizona Department of Education, community providers, local schools, churches and other agencies/organizations to coordinate access to appropriate services.  Service Coordinators partner with families and other professionals to identify goals and potential outcomes that will support the child’s continued growth, development, and learning. Service Coordination is available to three distinct populations of children and families:

Arizona Early Intervention (AzEIP)
Children from birth to three years of age who meet eligibility requirements for the Arizona Early Intervention Program.  For additional information, contact

Children with Special Health Care Needs
Children and youth, birth to 21 years of age who do not qualify for other services but have a chronic medical condition and/or developmental delay and have a need for information, support, coordination with multiple service providers and resources, and/or transition from the hospital, to the educational setting, or the work environment. Traumatic Brain Injury (TBI) 
A statewide system of supports and services available to children/teens who sustained a traumatic brain injury between the ages of 2 and 17 years and are within two years after the injury. The injury to the head must be documented in the child’s medical record and be related to one or more of the following conditions:

  • Observed or reported decreased level of consciousness
  • Amnesia
  • Skull fracture
  • Objective neurological or neuro-psychological abnormality linked to the traumatic event
  • Diagnosed intracranial injury, including lesion, concussion, contusion, laceration and hemorrhage and/or
  • Mild, moderate or severe brain injury
