OCSHCN contracts with agencies throughout the state to provide Family Resource Coordination in each of the 15 counties in Arizona through a comprehensive, coordinated, interagency, multi-disciplinary, and family centered approach. crawling baby

Family Resource Coordinators are responsible for securing and coordinating services across agency lines, and serve as an initial point of contact in helping families obtain services and assistance. Family Resource Coordinators partner with families and other professionals to identify goals and potential outcomes that will support the child/youth’s continued growth, development, and learning.

Family Resource Coordination is available to several populations of children and families:

Traumatic Brain Injury (TBI)
Children and youth who are survivors of traumatic brain injury sustained before the age of 18 may qualify for Family Resource Coordination.  Services are based upon self/caregiver report and/or medical documentation.

Spinal Cord Injury (SCI)
Children and youth who are survivors of a spinal cord injury sustained before the age of 18 may qualify for Family Resource Coordination. Provision of services is based upon self/caregiver report and/or medical documentation of a severance or severe injury to the spinal cord and or nerve roots caused by trauma.

Special Health Care Needs
Children birth to 21 years of age, with a broad range of disabilities or chronic illnesses may qualify for Family Resource Coordination. Such chronic conditions may necessitate adaptations, support, and or special services.
