The goal of the Medical Home Program is to promote and integrate family-centered, community based, culturally effective, comprehensive, coordinated health services for all children and youth with special health care needs. 

Our Medical Home initiatives promote purposeful partnerships with state and community-based agencies who provide services, care and health plans for children with special health care needs.

  • provide educational and advocacy materials
  • develop and facilitate medical home trainings
  • identify barriers and technical assistance to overcome them
  • identify community, state and national resources and disseminate information.
