Reporting Elevated Lead Levels
 

Health care providers are required to report blood lead levels to the ADHS. Physicians are required to report elevated blood lead levels, while laboratories are required to report all blood lead test results. 

Reports may be made by telephone, mail, fax, or by electronic transfer. Physician report forms and laboratory report forms are available. The statutory authority for reporting is found at A.R.S. 36-1672. The Arizona reporting rule A.A.C. R9-4-301 provides the reporting requirements.
