Thank you for your interest in becoming a registered sanitarian in the State of Arizona. Please fill out the application form, and mail it to the address shown below with a non-refundable application fee of $25.00 in the form of a check or money order made payable to Treasurer - State of Arizona.Arizona Sanitarians' Council

To ensure that your Sanitarian registration application will be reviewed by the Arizona Sanitarians' Council at their next meeting, your application, including college transcripts, must be received at least 7 days before the next scheduled meeting.
As per A.A.C. R9-16-402(C)(4), if applying under A.R.S. § 36-136.01(I)(3), arrange to have an official college or university transcript provided directly to the Council from each college or university. Copies of transcripts and sealed transcripts that have been provided directly to the applicant will not be accepted.
If the application is approved for sanitarian registration by examination, the applicant will be notified by mail of the next examination date and location. Upon receipt of this notification, a non-refundable examination fee of $110.00 must be submitted to the same address as a check or money order payable to the Treasurer – State of Arizona.
