Registered Sanitarian Application and Renewal
 

Applications 
A non-refundable application fee of $25.00 must be submitted with any new application. The fee should be submitted as a check or money order payable to Treasurer - State of Arizona. If the application is approved for sanitarian registration by examination, the applicant will be notified by mail of the next examination date and location. Upon receipt of this notification, a non-refundable examination fee of $110.00 must be submitted as a check or money order payable to the Treasurer - State of Arizona.  The downloads on this page are in PDF format.  In order to view them, it is necessary to have Acrobat Reader™ on your machine.

Renewals
Sanitarian registration must be renewed annually. A $10.00 renewal fee is due on December 31.
