is the Medical Home Project?
The Arizona Department of Health Services contracts with the American Academy
of Pediatrics, Arizona Chapter, to operate the Medical Home Project. The
Medical Home Project provides access to health care for uninsured children
from low income families who do not qualify (or are in the process of
qualifying) for the Arizona Health Care Cost Containment System (AHCCCS) or
KidsCare. The goal of the Medical Home Project is to increase access to health
care for Arizona ’s uninsured children. School nurses identify children who meet the Medical Home Project’s
eligibility criteria. The children are referred to participating volunteer
health care providers who have agreed to accept a predetermined fee of $5.00
or $10.00 as payment in full for each office visit. Specialists have also
joined the Medical Home Project. Children may receive free diagnostic
laboratory services, prescription medication and eyeglasses through the Medical Home
Project.

Is
my child eligible for the Medical Home Project?
If your child has an acute or episodic illness, he or she may see a doctor
through the Medical Home Project if he or she is eligible for the school free
lunch program. Also, in order to be eligible for the Medical Home Project,
your child cannot qualify for AHCCCS or KidsCare. Medical Home Project is not
available in all communities. Ask your child's school nurse if she or he
participates in the medical home project.

If
my child is sick, how can my child see a doctor through the Medical Home
Project?
The only way a sick child can be referred to the Medical Home Project is
through a school nurse, public health nurse or Head Start program. Not all communities participate in the Medical Home
Project.
If your child is in school, ask your school nurse if your community
participates in the Medical Home Project.
If your school age child has a younger brother or sister, ask your school
nurse if your community participates in the Medical Home Project for younger
siblings.
If your child is in Maricopa County Head Start, ask Head Start if they
participate in the Medical Home Project.
If your child is in Mohave or Pima County, ask a public health nurse if they
participate in the medical home project.
