Obtain a Certified Copy of My Child's Birth Certificate

  • A parent, grandparent or legal guardian can apply to get a certified copy of an infant’s birth certificate.

  • You can get the certificate by mail, by e-mail or in person. Government-issued photo ID and proof of relationship to infant required. Call or see website for details.
