Please Note: You must be able to present Valid Identification when applying for a Birth or Death certificate. 

Parking: Disabled parking spaces and some additional spaces for non-disabled parking are available close to the Office of Vital Records. These spaces are accessible from the south side of Monroe Street just east of 19th Avenue. Additional free parking is available one block east in our parking garage on 18th Avenue, midway between Van Buren and Adams Streets.

Many Arizona County Health Departments provide vital records services.  Please click on the link to the left for Local Offices to determine if you can be served in a local Arizona community.

Our customer service lobby is open from 8:00 a.m. until 5:00 p.m. However, in order to receive service, you will need to arrive before 4:00 p.m. All customers who arrive after 4:00 p.m. will be able to receive information only.  

The Office of Vital Records is responsible for maintaining and issuing certified copies of vital records, including birth, death and fetal death certificates and certificates of birth resulting in stillbirth for events that occurred in Arizona. The Office of Vital Records officially began recording birth and death events in July, 1909. However, it maintains a sampling of delayed birth records of Arizona citizens (from 1855) and death records (from 1877) from other sources.

Each state maintains its own vital records. If you are looking for a birth or death record from another state, you must contact that state's vital records office. Birth and death records for events that occurred in other states are not available from Arizona's Office of Vital Records.  For your convenience, we have a list of contact information for other states.

Marriage and divorce records in Arizona are maintained by the Clerk of the Superior Court in the county where the event occurred. Marriage and divorce records are not available from the Office of Vital Records.
